License Agreements (Terms & Conditions)

To complete the NetLibrary eAudiobooks License Agreement:

  • Download the license agreement Excel Icon (Word document; 119 KB)
  • Open the Word Document and type your library specific information into the highlighted fields (use the tab button to take you to the next highlighted field).
  • Once the highlighted fields have been customized, print the entire document.
  • Sign the document in the first page of the document in the "Subscriber" signature area.
  • Fax all 7 pages of the signed agreement to Mike Morey at 614-718-7163.

How to Order or Renew

Please complete the license agreement; then complete the order form and fax it to 612-625-3569 (except in the case of credit card payment; see below for fax number).

To pay by credit card, please complete the credit card authorization form and order form; fax all documents to our secure fax line at 612-626-5380.
